Richter's syndrome in a patient with B prolymphocytic leukemia.

چکیده
A63 year-old previously healthy man presented with marked lymphocytosis. Physical examination showed splenomegaly but no adenopathy. The blood count was 38.53109 leukocytes with 88% lymphoid cells, 170 g/L hemoglobin and 2463109/L platelets. The lymphoid cells were large with a single thick-rimmed nucleolus in a round regular nucleus. The chromatin was moderately dense and the cytoplasm was pale blue (Figure 1). Cytological diagnosis was prolymphocytic leukemia (PLL). Immunophenotyping of lymphoid cells by flow cytometry showed a B proliferation CD19+, CD5+, CD23+, CD24, Fm C7–, CD79+ with strong expression of surface IgM k. Cytogenetic study of blood cells was normal without 14:18 translocation and cyclin D1 was not expressed. The patient was asymptomatic for several months before developing, in a few days, enlarged, tumoral, cervical lymph nodes. Cytological examination of these lymphadenopathies showed voluminous cells: their nuclei had prominent nucleoli and very thin chromatin. The cytoplasm was deep blue (Figure 2). A diagnosis of immunoblastic lymphoma was confirmed by histological examination. This patient appears to have Richter’s syndrome, that is an aggressive lymphoma which occurred several months after the diagnosis of lymphocytic leukemia. We re-assessed the diagnosis of B-PLL with regard to the cytologic aspect of the lymphoid cells and the strong expression of sIg despite the negativity of FmC7. Richter’s syndrome occurs in about 5% of patients with chronic lymphoid leukemia, but to our knowledge, has not been reported in patients with PLL. Figure 1. Hodgkin's lymphoma variant of Richter's syndrome.
Chronic lymphocytic leukemia/small lymphocitic lymphoma (CLL/SLL) is low-grade malignant lymphoprolipheration, that has tendency to convert to a higher-grade neoplasm over time. More common is the development of a diffuse large cell lymphoma or transformation into prolymphocytic cell population. In rare cases, 0.1-0.5% of patients develop multiple myeloma or Hodgkin's disease. We present 65-yea...
متن کاملHodgkin lymphoma in a patient with chronic lymphocytic leukaemia--a rare presentation of Richter's transformation.
Richter's transformation of chronic lymphocytic leukaemia (CLL) to high-grade B-cell Non-Hodgkin lymphoma occurs in < 5% of CLL cases. Transformation of CLL to Hodgkin Lymphoma is a much rarer event and here we describe a patient who developed Richter's transformation into a Hodgkin Lymphoma presenting as rapidly progressive hepatosplenomegaly.
متن کاملIntravascular large B‐cell lymphoma presenting as Richter's syndrome with cerebral involvement in a patient with chronic lymphocytic leukemia
Intravascular large B-cell lymphoma (IVLBCL) is an aggressive non-Hodgkin's lymphoma which can present with B symptoms, rash, and neurological deterioration. Up to 10% of cases of IVLBCL are associated with other hematological neoplasms, including this extremely rare presentation of IVLBCL as Richter's transformation in chronic lymphocytic leukemia.
متن کاملCommon clonal origin of chronic lymphocytic leukemia and high-grade lymphoma of Richter's syndrome.
Patients with B-cell chronic lymphocytic leukemia (CLL) infrequently may develop high-grade B-cell lymphoma, or Richter's syndrome lymphoma (RS lymphoma). Such lymphomas differ from the original leukemia in both histology and clinical behavior. Studies seeking to define the clonal relationship between the cells of the two malignancies in any one patient have yielded conflicting reports. We exam...
متن کاملExpression of A-myb, but not c-myb and B-myb, is restricted to Burkitt's lymphoma, sIg+ B-acute lymphoblastic leukemia, and a subset of chronic lymphocytic leukemias.
The A-myb gene encodes a transcription factor that is related both functionally and structurally to the v-myb oncogene. Following our observations that A-myb is expressed in a restricted subset of normal mature human B lymphocytes, with the phenotype CD38+, CD39-, slgM-, we have now investigated the pattern of A-myb expression in neoplastic B cells representating the whole spectrum of B-cell di...
